Adult social care teams are under increasing pressure from rising demand, overdue reviews, complex community support arrangements and stretched operational capacity.


In many areas, support packages are only being reviewed when they are unstable - leading to reactive intervention from operational social work teams, brokerage, providers and commissioners.


At the same time:

  • Overdue reviews continue to grow

  • Teams are stretched

  • Providers are managing increasing complexity

  • Support plans do not always reflect current reality

  • Co-ordination across services is fragmented.


Commissioners need to think differently about how they operate to meet future demands, flexible arrangements and how they review their own capacity to support complex community-based packages before they escalate further.


Empowered 2 Support Ltd is currently exploring a small pilot using our ISF-style principles to support:

  • complex support package reviews

  • person-centred support planning

  • provider co-ordination and alignment

  • improved visibility of risks and delivery issues

  • stabilisation of community support arrangements

  • reducing repeated operational escalation and intervention.


The approach is designed to work alongside existing teams and providers -

Supporting councils and NHS with additional capacity, potential efficiency savings, improving people's outcomes all without any large-scale redesign.
